I just noticed a glaring error in my post.

I corrected the urls and the information and question are now accurate.

Any answer on why/how sef advance could interfere with the url processing when there is not sef_ext.php in the directory?

This issue is holding up a 1.0.x to 1.5 migration.

Jay: I think I just fixed this. Please download the latest SEF Advance 2008 v2.1.2 and test. If that doesn't fix it send me the info.
