Using Joomla 1.0.12 with iJoomla.
Installed the demo - instantly broke my site.

Please follow the installation instructions from documentation! Replace your .htaccess with the one included in SEF Advance zip package.

Hi Saka,
Better - almost there. some of the urls are messy. Example:
Falklands 1982 / 1984-85 - url -
http://www.rnmedics.com/medic-musings/falklands-war/falklands-1982-%10-1984%1185/
I've tried configuring exceptions thus - ?|!|/|- this doesn't make any difference. Any ideas, please?
Regards,
Mike

You can't except "/" and "-" from URL encode because / is used as directory separator and - is used as word separator.
