The problem was that user renamed some of the SEF Advance system plugin files which made SEF Advance unusable.
Note to other users: never rename any of SEF Advance files, never modify any of the core files (if you don't know what you are doing) and install / uninstall SEF Advance only as described in documentation.
It should work. Make sure that's the non-sef URL when you disable SEF. If you can't get it to work send a mail with access info and I will take a look.
